Summary

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.

Consequence

During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.

Remedy

Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.

Summary

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."

Consequence

Driving with a deactivated stability control system increases the risk of a crash.

Remedy

Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

Consequence

The wiring harness connector for both lights has had water accumulate in the connector which causes the rear tail lights to fail. Both of my tail lights have failed, and the cause was corrosion inside the wiring harness connector. I had to replace the wiring harness and connectors on both sides, driver, and passenger And I had to pay out-of-pocket. The dealer said that this problem is common on 22 sienna's and I have seen other 22 sienna's with out their driver and passenger rear running lights burnt out.

SERVICE BRAKES

For over a year now, I have been experiencing a random issue in my van with the brake system. It feels like a slippage when I hit the brakes to slow down and it will randomly feels like something slips and just give gives out and gets loose and like you are free falling until the brakes grab and actually stop. I have had it in the dealership twice to fix this issue. Last year they reprogrammed the brake actuator assembly control module, and it worked for a while, but then once again after several months, it started again and the second time I took it in they drove it around to try to get it to mimic the problem, but was not able to so Couldn’t do anything about it and just said to watch it and see if it became more frequent. They also said that once you have the actuator reset the computer shows updated and can’t redo it.The problem with this issue is that it is random and sometimes it’s less and sometimes more and I can’t give specific dates because of the randomness of the problem. I have never not had it to stop, but I am including my two invoices from Toyota of Richardson where I’ve had it seen as well as the research that I have looked up in different forums where other people are describing the very exact same problem with their vehicles. [XXX] . It is scary because you don’t know if it’s going to stop or not but so far it has. [XXX] . There is an issue, but don’t know how to permanently solve it or know for sure what it is. The forum does explain some different things because of the vehicle being hybrid, but who knows whether that’s it. Please look into this and advise because I do not wanna wait until this is a problem and I have a wreck because I am not able to have my brakes work Correctly. P.s. Brakes release briefly when slowing to a stop. While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This has resulted in near collisions with cars, bikes, and pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over at least 6 months of ownership. No warning lamps, error messages, or other related symptoms from the vehicle either before or after occurrences of the problem. Has been taken to the dealership for an update to the vehicle but it didn’t fix it. There are multiple reports online and multiple lawyers talking about this.

Pre-Collision System Malfunction, Hybrid System Malfunction, AWD System Malfunction suddenly when restarting the vehicle. Towed to Toyota of Downtown LA dealership. Toyota of Downtown LA Service tells me that the CPU on my relative new Sienna 2022 was fried from jump start when Sienna starting-battery was low. It was done by a AAA professional, I doubt that is the reason for the fried CPU. I believe Toyota engineers would not design/allow the CPU vulnerable to being fried from a jump start. This is a horrific design flaw if it is this way. This happens now, just 6 months after the 3 year warranty coverage. Toyota of Downtown LA Service is telling me it is going to cost me $2900 to replace this CPU. I find this unacceptable.

The Entune 3.0 System governs navigation, calls etc. The system on my car has been replaced 4 times and still does not work. This is a life safety issue as the system grabs phone calls while driving without being able to answer. It also makes it frequently impossible to use hands free on the phone as it grabs calls. In order to avoid it grabbing calls I have to disable bluetooth on my phone which then causes my phone to not work in handsfree mode (ok google etc.) Furthermore the backup camera does not always work. Finally, a Toyota Service Manager told me in his oppinion 25% of the entune units are "bricked," non functional and need to be replaced. Toyota has fired according to him Panasonic as the supplier and now has a new provider.

This failure has been independently observed by myself, wife and mother. The vehicle did not display and warnings prior to the failure. I am writing to formally raise a serious and unresolved safety issue with my 2022 Toyota Sienna. The braking system on the vehicle behaves abnormally: when the brake pedal is pressed, the van does not stop immediately. In fact, it momentarily accelerates before decelerating, an extremely dangerous malfunction that has nearly led to multiple accidents. I have brought this issue to your attention several times: • In 2022, I contacted O'Regan's Toyota, the Halifax dealership, which has been unable to diagnose or repair the issue to date after several visits. • In 2023, I contacted Toyota Canada directly. I was referred to the dealership in Halifax. • The Halifax dealership has been unable to diagnose or repair the issue to date. • Despite the unresolved nature of this problem, the dealership has recently asked to charge me diagnostic fees for the same issue, one that remains unsolved and is clearly a manufacturer-level safety defect. This is not a minor inconvenience; it is a fundamental failure of the braking system and presents a critical safety hazard. The fact that this issue persists without a resolution, and that I am being asked to pay diagnostic fees for a problem that Toyota has not addressed, is unacceptable.
